FOP Member License Plates
FOP Associate Member License Plates
In cooperation with the Ohio Bureau of Motor Vehicles (BMV), the FOP offers a special FOP member only license plate and a FOPA member only license plate. These plates can be ordered at any local BMV Office, but first you must receive an approved application from the Ohio State Lodge. In order to approve your application, please fill out this form. You will receive your approved application(s) in the mail, which you can complete based on the style of plates desired, and take to any local BMV.
